Breaks npmjs.com login

This extension breaks logins on the npmjs.com site at the OTP verification step.

Steps to reproduce:

• Log into new account with username / password (this triggers the sending of an OTP)
• Enter OTP
• Instead of logging in, you are redirected to a 404

I have narrowed down the issue to this specific extension. I see that there are multiple similar issues reported; possibly this is related.
