History tab for concepts and terms #532

History of concepts and terms do not capture all changes (eg added relation). 

Check also what fields are displayed.

The history tab in term browser is not refreshed after adding the relation, 
works after closing and reopening term browser. This will be fixed. Please 
confirm, if there is any actions which is not captured at all?

Complex issue to check. History track is generally very good now. Here some 
observations for final discussion.

1. concept-create: when a concept is created, it is no shown anywhere in the 
History tab who created it.  
2. concept-delete: in table of changes only shows the user who deleted it. No 
creator. 
3. relationship-create: it is tracked as concept-edit-relationship. Please 
check. 
4. relationship-edit: it is tracked as concept-edit-realation both when you 
edit the relationship and when you edit the other concept in the relation.
5. ext-source-create: not sure to what this action is supposed to correspond.
6. image-source-create: same as point 5. above.
7. term-note-create. as above.
8. term-note-edit. as above. 

I also realized that actions  on terms, including creation/editing of new 
terms, only appear on the History change of terms, not on the History of 
concepts. I was wandering if at concept level one should be aware that 
something has appened to its terms...
