If you use a light source repeatedly and drive the quantity to zero, a slash appears over the token. If you right-click to select a different light source to use, it doesn't respond until you close and reopen the HUD.
(I've found the cause of this one and have a two-line fix pending. This will remind me to deliver the fix.)
