This seems easy/fun, I'd be down taking this on sometime in the evening in the near future.

I whittled this down from about 20 tabs so do with it what you will. I personally like the simple ones but I did see some form-esque ones somewhere too?
It's a very nifty GitHub feature! We just need to decide what the template should look like. (I imagine this will become clear after we've been using GitHub issues for a while, so no rush here.)
