Rearrange and name fields on the change QTL/marker page #123

This issue is associated with issue #66
I think it makes more sense for the user to choose the marker type first and then add the track title.
1) Move the dropdown list of different marker types above the Track title field.
2) Above the marker dropdown add the words "Choose Marker:" similar to the field above where it says "Track title:"
