How many Notarized Waiver and Consent to Petition for Guardianship forms do we produce?

Right now, assumption is that we only produce 1, and that it has space for both parents to notarize, and only if the status is "yes" or "unknown" for whether they consent.

But we could theoretically produce one for each person had had custody over the last 60 days, or we could make a standalone version of the form and ask them each to produce this.

Either way, we need to update the instructions to explain this form since most of the work will happen later (e.g., the parent has to sign in front of a notary and acknowledge the fact that they're waiving certain rights)

We can only produce these forms when the petitioner knows in advance that the interested party consents; otherwise this should be dealt with in the instructions (this is to avoid pressuring the interested person or parent to sign without enough information--they can do it at the hearing)
