The letter to inspectional services requests a phone call and says that the user's number is listed below, but the phone number is not a required field so the letter isn't very useful if this field isn't filled out.

Contact information is not currently required, even when suing landlord

Strongly encourage user to put a phone number/email (that they can access for notices) of a family or friend if they do not have one. Look to MADE for example.
