Update language on "bad_conditions[i].details[j].description" to remove "optional" and make more clear in beginning of label that it is optional #540

The usability audit found that users were under the impression that they must fill these questions out despite stating that they were optional.

Update the language so that it is clear up front that they only need to fill it out if they would like.
