Create template - examples for why a tenant couldn't get response ready and file in time #606

It would be useful to provide examples of why a tenant might have been unable to prepare a response and file in time.

Create a collapsed template and add it to the question with the id of reasons_unable_to_prepare_exists.

There are many examples of collapsed templates in the yml files.
