Layout of Customer fields could be improved. #13

The layout of the customer fields could be improved to be a bit more
'horizontal'.  For example, City/State/Zip could be on one line with the
State field being smaller.  This is low priority of course (and not part of
the class requirements).

Layout match UI initial mock-up document. Horizontal distribution need to be 
tweaked.
