Add “Please follow the instructions below.” to Abstract Measures #9

On both abstract measures, above the line at the top of the page, add “Please 
follow the 
instructions below.” Add it starting at the left margin. Put it below the 
level of the page number, 
which is above the line at the top of the page at the right margin.
