Add comment tool

Overview:

This enhancement will enable the user to add a comment on a feature (common 
comments that users wish to make include incorrect spelling, wrong placement, 
etc).  This functionality is a subset of editing in enhancement 2. Information 
will be collected from the user on: which datalayer is at issue, which feature 
is at issue, comment, timestamp, user’s name (optional), user’s contact 
info (optional).  If the edit does not successfully enter the database the user 
must be notified. 

Detailed Description:

The configuration of this tool will indicate:
• The GIS point layer into which the comments will be stored.
• The columns of the GIS point layer which will store the users comment, 
contact info, timestamp, name and datalayer at issue.
• Optionally: the maximum length of the field so that a user will not be able 
to submit a comment longer than the underlying database field size
