Referral Form allows requests to be made with no information

What steps will reproduce the problem?
1.signed in as mary
2.clicked on new referral
3.only entered a beginning and end date

What is the expected output? What do you see instead?
it accepted just that limited information. i was not told that i need to enter 
in more than just the dates
