Patient's name not displayed, other UI issues when creating a new request

This is related to Issue 57, and adds more UI problems

What steps will reproduce the problem?
1.Login as mary789
2. Choose New Room Request
3.Search for Jane Smith
4. Click on Jane Smith's link at the top of the results.
5. Click on Create New Reservation at the bottom of the detail results

What is the expected output? What do you see instead?
 The date entry boxes at the top refer to a "report" - but we are not selecting a report here, we are selecting dates for a room request

Also, patient's name should be displayed!

Finally, the page comes up with a "Referral was unsuccesful" message before we 
even have a chance to select dates and create a referral.

Please use labels and text to provide additional information.
