Patient information is not prefilled when making a room request for a known family #75

What steps will reproduce the problem?
1.Login as Mary789
2.Choose Room request
3.Enter Jane Smith for family information
4. Click through to the point where you can choose Create Reservation

What is the expected output? What do you see instead?
We should see the patient information, filled in with information extracted 
from the database.
Instead, the form is empty.

The form is not fully empty. The PatientNamewouldprefilledhere is the value in 
that field. However, the other fields are field labels and are void of values.
