Explore removing global declare svgns and xlinkns variables #398

We currently automatically declare global variables 'svgns' and 'xlinkns'
variables to aid document authors. However, this has the end result that
*.svg files that use this won't load in native browsers without the SVG Web
library, which is bad. Explore removing this support and updating all the
docs and samples to simply declare these variables themselves.
