Reorganize the Spec #140

Problem

The separation between DSNP spec and chain level implementation was not clear or easy to see. Link to GitHub Issue: Closes #137, #138

Solution

Re-organize the spec with a focus on keeping the implementation retails separate. Do not change any requirements in the specification

Change summary:

Create new menu structure
Split out Activity Content to a separately versioned spec
Create new DSNP Implementations section with DSNP: Ethereum
