Improve wider medium screen responsive layout #68

The manual is best viewed with a minimum screen width of ...

It is recognised the existing template displays poorly when the display width is significantly less than the content design width of 700px on a 1100px screen. This template is particularly affected by the poor display issue because:

Displaying the hamburger menu icon instead of TOC on wider medium screens displays improves the responsive layout by displaying content closer to design width. It is considered with this change the responsiveness would be so similar to other sites that the warning about best viewed minimum screen width would not be required.

The HTML option was added in Sphinx 1.8.0b1 (released Sep 2018). The recommendation in Read The Docs Documentation is adding custom css to the theme via the html_css_files option in conf.py.

Tested in dev and it didn't change the appearance.
