6 years ago Found a source: http://wowwiki.wikia.com/wiki/Dungeon_Set_2/Pants,_Shoulders_and_Boots?oldid=326478 (November 15, 2006, patch 1.12) The Top Piece of Lord Valthalak's Amulet will be found on Theldren's corpse after the fight is complete. In the event of you miss any part of the Amulet, Bodley will be able to offer a new one.
I've tried to search for info for that issue but i had no luck with it.
Ok, As a part of dungeon 2 tier quest chain this quest (The Right Piece of Lord Valthalak's Amulet) requires to kill certain summoned bosses (Kormok, Isalien - depends) and the item required to turn in that quest is Lord Valthalak's Amulet. Lord Valthalak's Amulet is created by combining Right piece (which you get from current boss) and top + left pieces (which you get from previous bosses). Somehow i forgot to get TOP piece (Thelrden drops it). And (as support team asked me to do) i reloaded my computer 3 times, deleted my wdb folder and killed Theldren again 2 times. And at this point i'm totally sure there is no way to get TOP piece of the amulet again if i've lost it. I'm not sure, but probably you can finish "the Challenge" quest without showing Top piece of the amulet. I'm asking for any way to get that item (The Top Piece of Lord Valthalak's Amulet).
