Update evaluated-output datatype #52

Also fixed the short and definition descriptive text for evaluated output to be associated appropriately to the evaluated-output extension in context of the InsightDetail

Ready to merge - change of evaluated-output data type from Reference to Attachment; Version updated to a new major version since this is a breaking change from prior version.
