workflow updates #9

update README.md
- some formatting clean up
- added some clarifications
- updated the workflow description to a model the requires less movement of complete files
- update the specification template to include a state indicator

I'm okay with the changes, so +1 to that. At some point I want to take the time to read through this in more detail and propose some updates, but we can consider that a separate issue.
