Coursepack issue: page numbers/navigation #196

Currently, there are no page numbers present in the coursepacks. We need the original s, but also pagination for the items in the coursepack, too, as a finding aid. These should be available on the table of contents, too. We are imagining scenarios when users are printing a pdf of the coursepack and would want to turn quickly to a page. Also, in an ereader, where a link would be provided from TOC to the item.
