convert indexes to chapter #19

Add the following to the index of each chapter, with the title altered

Hoping this will help get the menu to show each chapter (and not just the first one). The others are there if you click through with arrows, but not appearing in menu until you're on them

Did indeed sort out the chapters appearing in the menu but has brought up a new issue:

[x] get menu to order the chapters by number rather than alphabetical (using the chapter numbers in the index headers)
