The title text is so small.

You don't have an input box for bank name, there is nothing below it

ID should be capitalized.

What is 'id proof' ? That is a very vague statement, no clue what type of information you're looking for especially when a user has to upload a picture.

When very sensitive information is being requested that process is usually the focus point and also requires multiple screens. Not just one upload box

When clicked on the upload button you can only choose documents, while the upload box mentions that images are also allowed. You can' access your gallery.

Restricted only for the pdf file for documents.
