[lcc_members] Add membership start date in the membership registry wizard #233

Add a field to indicate the actual membership start date in the membership registry wizard from partner form view

Finally I did not add a date field in the wizard because it is complicated and there is a risk to break the online membership process. Instead I simply changed the date field attribute readonly in the partner form view, so that the user can change the membership date afterwards.
