Dropped quest starters not dropping

I've noticed while I've been leveling that many dropped quest items which other people are able to loot, simply do not drop for me and I have no idea why. I've deleted my cache, I don't know what else to do.

Just off the top of my head there are these three:

I've killed these NPCs multiple times trying to get the drops with no success. The last of which is the start of a very long chain.

I got Ungulas fang on first kill, I got burning legion missive on first kill.

Thunderlord Clan Artifact is really low drop rate.

I had to kill razorsaw 6 times to get it,And ogres has 3% rougly to drop it.Its not that comon.
