Set Upload Reading True as the default #180

Default to uploading G5/G6 readings to remote service. Users tend to forget to click upload following Transmitter change and then are not getting glucose readings uploaded.

The preferred solution would be to enable user to change transmitter without deleting CGM and/or move upload selection to the remote service module.
