Create form for CBOs to complete before advancing to view applicants.

Modal form on top of Applicants Selection page using this content:https://docs.google.com/a/boston.gov/document/d/1ojFL-CNhOYbg_7A89tmX5C8qZl7c2gHWa6OExd_Fxe4/edit?usp=sharing

If they want to select or click on applicants before completing form, they cannot. If they complete form, small text on applicant viewer indicates their form has been submitted.

@arouault Hello - can you grant permission to look at the form?
