Allow a user to request verified status independent of a municipality #219

Resolves #211 .

Why is this change necessary? State and regional users will want the ability to request verified status independent of a specific municipality. Specifically, it's important that they can create developments outside of their home town.

If you sign up as an unverified user, the "Request for verified status denied" message shows. We shouldn't show this message if a user never requested verified status during sign up.

When denying verified status to a state/rpa employee, you get a null version of the municipal membership message.
