Create alert entry view #2

The alert entry view allows dispatchers to enter the following information:

Call location
Chief Complaint
Patient age
Patient sex
Whether patient is conscious
Whether patient is breathing normally
Notes

Once this has been entered, the controller should use the MCEMS API Client to post the alert to the API server so that it can be forwarded to Active911.
