Update NGINX Proxy #82

Hi,

This is a re-submission of PR #63.

These changes reduce complexity of the NGINX proxy, enables dynamic configuration of server_name via .env file, fixes NGINX proxy_pass location (#81), enables access log by default, and maintains the existing NGINX configuration settings. NGINX container build is no longer required.

Sorry, I just noticed that this project has no LICENSE (#65) so I'm closing this PR.
