Remove APACHE pre tables #15

There are three "pre" APACHE tables in the documentation:

apachePrePatient
apachePrePatientResult
apachePrePatientVar

None of these tables will be in the final release - do we want to describe the process of generating them (and their eventual transformation into the final APACHE tables) - or do we want to simply not discuss them in the documentation?

I have removed these from the public documentation (http://eicu-crd.mit.edu). Closing this issue for now, but we can open a new issue if we decide more information is needed on how the APACHE tables are derived.
