Update Table 2 with latest database (v1.4) statistics #15

MIMIC has changed since Table 2 was generated, so the numbers will need to be updated. The code is contained in an Jupyter Notebook, so this should be straightforward: https://github.com/MIT-LCP/mimic-iii-paper/blob/master/example-patient.ipynb

v1.4 not released in time for publication of paper, so updates not necessary.
