Unbranding: Create a config folder #1342

As part of our effort to move towards an "unbranded" platform, we plan to add a config folder to the repo. Users are expected to clone a separate configuration repo (in our case https://github.com/MIT-LCP/physionet-config) into this folder.

As work progresses, some or all of https://github.com/MIT-LCP/physionet-build/tree/dev/deploy and https://github.com/MIT-LCP/physionet-build/tree/dev/physionet-django/physionet/settings will be migrated to the config repo.

The Django CookieCutter repo may be useful for reference. e.g. this json config file https://github.com/pydanny/cookiecutter-django/blob/master/cookiecutter.json
