When someone requests access to a "contributor-managed" page, the data contributor needs to approve or deny the request via a page at (e.g.) http://127.0.0.1:8000/access-requests/demoselfmanaged/1.0.0/1
The page needs a major clean up. For example, the page currently displays incorrect information about the user requesting access. This pull request implements some quick fixes to the page at the request of @xborrat
