SQL changes TBD for revisions

• [x] Change dialysis exclusion from first week to before 12 hours (i.e. on or before ICU admission)
• [ ] Change AKI definition to include dialysis as AKI level 3 (specifically add it to the aki_48hr clause in aki.sql)
• [x] change nephrotoxic exposure to [-12, 12] hours (sql/nephrotoxic_agents.sql)

I believe I made the first two changes already - in dialysis.sql and AKI.sql
