Intermittent post stream problem

It seems like posts sometimes don't get included into the post streams correctly. For instance, I am a follower of the Dance Party shake. Most of the time posts posted in Dance Party show up in my main feed, but sometimes they don't. In that case they do show up in Incoming, as well as the main Dance Party stream. Dance Party is just an example, this happens with individual users as well. I don't know how to reproduce this issue. [captured from the initial bugs sheet, from user bjrn]

I have an example now. I made a shake (http://new.mltshp.com/test-long-title), which contains two images http://new.mltshp.com/p/1EN and http://new.mltshp.com/p/1F4

The former doesn't show up in my main post stream (what you get on the main page), but the latter does.

Since the beta site is gone: In my friend shake I can see https://mltshp.com/p/1BPFL and https://mltshp.com/p/1BPFJ (both from the dance party shake). But when I check the dance party shake itself, there is a post (https://mltshp.com/p/1BPFK) that doesn't show up in my friend shake.

All three posts are posted to tonyb and danceparty, I am following both shakes.
