This merge adds a number of bug fixes, enhancements, and minor features to the subglacial hydrology model and it interaction with the rest of the model:
option to make beta a function of effective pressure (e.g. for a Weertman basal friction law that includes N)
correct pressure variable values under floating ice and open ocean to allow the SGH model to be used for marine ice sheet problems
two new options for getting basalMeltInput from 1) the thermal model or 2) the basal heat fluxes
remove the vestigial adaptive dt option from the subglacial hydrology model
improve performance of SGH global reductions by using an array
move SGH to after the thermal solver within the time step
correct the 'sfc_air_temperature' thermal init option to honor the PMP temperature
