Add a menu that describes the UI (including the gestures)

Based on feedback on r69 of the codebase: please can we add a menu that
describes the user interface, including gestures, of the application.

Thanks to Kevin McDonagh for requesting this.

Comments:
   * Given the current minimal UI this should be easy to do.
   * We will need to pay more attention to making the UI useful and
practical for a range of users, including those with and without the
ability to read the screen with their eyes.

I've added a basic help activated by the menu button. There will be further 
improvements to the help and menus, however I'll close this issue now since the 
fundamental requirement seems to have been addressed.
