Currently, if you want to update a table made with MDTOC, you have to manually remove the table and re-add the **MD-TOC** text to the file. That is tedious and not conducive to quick development.
Describe the solution you'd like
[x] Replace the **MD-TOC** text with two comments that mark the beginning and end of the TOC
[x] When mdtoc is called:
[x] Remove existing ToC between the comments
[x] Regenerate the ToC and place between the comments
