Implement monitoring hooks #14

Implement hooks so it's possible to develop monitoring plugins (ie. for New Relic APM, Datadog APM, Active Support instrumentation, etc).

Most application performance monitoring money patches Net::HTTP internals (😢) so there is no real reason to provide monitoring hooks.
