UI issues when scanning QR code for address book #199

What steps will reproduce the problem?
1. Go to the address book and press the "Scan address" button
2. Scan a QR code encoding a bitcoin: URL with a label and amount

What is the expected output? What do you see instead?

The "Add address" dialog pops up with the scanned address, but does not show 
the label as the default contents of the "Name" field - I would expect it to do 
this.  Furthermore, more buttons have appeared at the top of the display - the 
"Paste" and "Scan address" buttons have been duplicated.  If the operation is 
repeated, the "Add" button gets duplicated as well.

What version of Bitcoin Wallet are you using? On what Android version and
what phone? Don't just state 'most recent', this is not enough info.

Bitcoin Wallet 3.13
Android 4.2.2 on Galaxy Nexus
