html2pdf needs table of contents generator and pagination abilities #1567

Using html2pdf in Plone to turn a multi-section/multi-page manual into a
single/printable PDF needs some improvements. Namely a table of contents (with
page numbers referring to PDF pages) and a new page inserted between sections.
