[Docs] Several FAQ in PHPMAPSCRIPT-INSTALL-HOWTO are obsolete #172

Several of the FAQs in the PHPMAPSCRIPT-INSTALL-HOWTO date from pre-3.5 days and 
do not apply to the current version of MapServer any more... so they just 
confuse the users with incorrect information.

Also, the installation instructions are quite likely obsolete as well.  Same 
applies to all our HOWTOs I would think... so I think that we need to take some 
time to review all our HOWTOs (on a rainy weekend ;)

Jeff, do you think you can review/update your HOWTOs for the above problems and 
any other problems before the 4.0 release scheduled for mid-July?

I have updated my 3.7/4.0 docs.  A mapcontext-howto now also exists (although 
it is not posted yet).
