Markup style clean-up and 2 typos #165

While I came to fix 2 typos (1, 2), I quickly realized I haven't been following the markup style guide very well in my previous contributions.

I revised the whole document, and found that these were common style notes that hadn't been followed:

Start and end tags for paragraphs and sectioning elements go on their own line.

Leave a single empty line between sections or before an h5 heading.

This PR should fix all the occurrences where that isn't followed. ^^

Looks like I'm not very good at following my own rules! Or maybe, I'm not very good at writing down rules that describe the way I do things?!

Either way, best to be consistent & match the styleguide. Thanks as always.
