Various examples #166

I have more example-related stuff to add, but I wanted to make this pull request so the stuff in here can be given a quick review, if possible.

In particular, I have examples that are using GitHub Pages tile sources from repositories I've created at: https://github.com/Earthmarks/FiskMississippiMapsStreamCourses https://github.com/Earthmarks/TiledArt-Rousseau

These repositories contain tiles at multiple zoom levels, while remaining within the recommended maximum size of a GitHub repository (1GB; though they tolerate larger repos, they seem quite keen on that limit).

I thought it might be a good idea to fork them into Maps4HTML and set the forks up on GitHub pages, as then the examples that use them could be modified to use the Maps4HTML copies, avoiding the dependency on my business GitHub account. (The imagery is all in the public domain.)

Oh, this is great! (Caveat: haven't actually run any of the code.)

I agree it makes sense to fork the tileset repos into the Maps 4 HTML repository for use here. Maybe add a readme (or at least Github repo description) to the second one. You should have the permissions to do that @NickFitz — ping me if you have any problems.
