Add custom fields for users, roles, and circles #429

Need to be able to log who is the coach of each individual (they don’t have line managers). This would be something like a data field for each user. Perhaps we could have custom fields for people and one data type is another person. Staffan@Comprend

I've modified this slightly to be encompass adding custom fields not just to users, but also to roles and to circles. Adding this to users is still the highest priority though. Then roles, then circles. Might do this separately for users, roles and circles in the end, but wanted to keep track of all of these together.
