Recreate the PPE registry Form within the platform #9

Description:

As this is the currently most used existing Form, it has to be the first to be implemented in the platform so we can start transitioning to it.

Acceptance Criteria:

PPE Registry Form ready and available implemented in the platform with the same functionalities as before, at least.
