Work around errors in virtual entity providers:

not supporting top, count, page or distinct attributes
not supporting <order> element
returning data with attribute names in different case from metadata logical names
returning data in different types or with length exceeded that described in metadata

Still fold operations to FetchXML where possible but also keep native SQL 4 CDS operators in case virtual entity provider ignores the instructions.
