I experienced this issue as well. I was able to recreate it by having two players on the server with separate homes and then having one of the players disconnect and reconnect multiple times in quick succession. Looks like the homes of the player disconnecting and reconnecting overwrote the homes of the player who stayed connected to the server. From this, it seems like the issue occurs when a player connect and disconnect with my first suspect being the SaveFile class
Would someone be able to verify that this method recreates the issue?

I can confirm that this is an issue, and it does seem to be related to that class.
Unfortunately, this mod is fairly badly-written from what I can see — and it also appears to be abandoned. Might be worth creating an alternative.
When running on server-side on a dedicated fabric server, the homes will sometimes cycle between players or just stop working until relogging. When it stops working it just returns something along the lines of "Commands.home.failure" in red text. This is sometimes fixed by relogging. Sometimes it isn't and you need to clear and reset your homes. As an example, recently my /home was lost and was showing autofill options for other players homes. They had two homes set named "h" and "v" which now are the only homes that I can use and allow me to auto fill with tab. When these homes became available for me to use, the other player had their homes "deleted" and could no longer use them. It is as if their homes were literally traded over to me.
Recreate: Im unsure of exactly what causes this but homes worked fine for a while before they started swapping and bugging out. Having a few players on a server for a while using /home and /delhome should show the problem eventually. It may have only started occurring after a server restart but I am unsure.
Conditions: Running Fabric Server from "Fabric installer 0.6.1.45" for 1.16.1
Mods: (Server is vanilla compatible)
Datapacks are installed but were installed after problems had already occurred. They Are: Vanilla Tweaks
There are currently twelve players that play on this server. All other commands from my commands appear to function properly including /warp and /back and /tpa.
Server Host System (This is my own personal PC that I port forward from my network. I'm not sure if this is relevant though)
Server runs at 20tps constantly without any evidence of lag.
